1. Pathophysiology

Tuberculosis spondylitis, classically known as Pott disease, results from extrapulmonary dissemination of Mycobacterium tuberculosis. The organism initially establishes a primary infection in the lungs. From there, bacilli enter the bloodstream during a phase of bacillemia and travel to the vertebral bodies. The route of spread is predominantly through the Batson paravertebral venous plexus, a valveless venous network that allows bidirectional flow and preferential seeding of the vertebral cancellous bone, which has a rich blood supply.

The infection characteristically begins in the anteroinferior aspect of the vertebral body, near the subchondral endplate. This is because the arterial blood supply to the vertebral body is concentrated in this region, making it the most vulnerable point for bacterial lodgment. Once established, the infection causes caseous necrosis of bone, leading to progressive destruction of the vertebral body. The avascular caseous material cannot mount an effective immune response, allowing the infection to spread beneath the anterior longitudinal ligament to involve adjacent vertebral bodies. This pattern explains why TB spondylitis classically involves two or more contiguous vertebral bodies with destruction of the intervening disc space (paradiscal pattern).

As the anterior vertebral bodies collapse while the posterior elements remain relatively intact, the spine develops anterior wedging. This creates the hallmark gibbus deformity, a sharp, angular kyphosis that is clinically visible and palpable. The thoracolumbar junction (T12-L1) is the most commonly affected segment because it is a transitional zone with high mechanical stress and good vascular supply.

The caseous material and inflammatory debris can track along fascial planes to form cold abscesses. These are called "cold" because they lack the classic signs of acute inflammation (rubor, calor). In the thoracic spine, the abscess can present as a paravertebral or mediastinal mass. In the lumbar spine, the material tracks along the psoas muscle sheath, producing a psoas abscess that may present as a groin or thigh mass. This pattern of abscess formation is highly characteristic of TB and distinguishes it from pyogenic infections.

Neurological compromise occurs through several mechanisms: direct compression of the spinal cord or cauda equina by caseous tissue, abscess, bony sequestra, or disc fragments; vascular compromise of the anterior spinal artery causing cord ischemia; and in chronic cases, constriction by fibrotic granulation tissue (described as the "disease of activity" versus "disease of curing"). This distinction matters because neurological deficits from active disease respond to anti-TB therapy, while deficits from fibrosis and bony compression typically require surgery.

Best Initial Test: The workup begins with a plain radiograph of the affected spinal segment. On X-ray, the classic finding is destruction of two adjacent vertebral bodies with collapse of the intervening disc space. You may also see anterior vertebral body scalloping, a paravertebral soft tissue shadow (suggesting abscess), and kyphotic angulation. However, plain films are limited because radiographic changes do not become apparent until 30 to 50 percent of vertebral bone is destroyed, meaning early disease can be missed entirely.

Most Accurate Imaging Test: When clinical suspicion is high and radiographs are equivocal or you need to assess the full extent of disease, MRI with gadolinium enhancement is the study of choice. MRI provides unmatched soft tissue resolution and can demonstrate marrow edema before bone destruction is visible on X-ray. The characteristic MRI pattern includes: T1 hypointense and T2 hyperintense signal in affected vertebral bodies, involvement of two or more contiguous vertebrae with disc destruction, subligamentous spread of inflammatory tissue, and rim-enhancing paravertebral or epidural abscess. MRI is also essential for evaluating cord compression and guiding surgical planning.

Confirmatory / Gold Standard Test: The definitive diagnosis requires tissue sampling through CT-guided percutaneous needle biopsy or open surgical biopsy. The tissue is sent for:

Histopathology: Caseating granulomas with epithelioid histiocytes and Langhans-type multinucleated giant cells.
AFB (Acid-Fast Bacilli) stain: Rapid but has low sensitivity (approximately 25 to 30 percent).
Mycobacterial culture (Lowenstein-Jensen medium or BACTEC liquid culture): Remains the gold standard for species identification and drug susceptibility testing; requires 4 to 8 weeks for results.
GeneXpert MTB/RIF (Xpert Ultra): A nucleic acid amplification test that provides rapid results (within 2 hours), detects M. tuberculosis DNA, and simultaneously identifies rifampicin resistance. This has become the preferred rapid diagnostic in resource-limited settings and is endorsed by WHO for extrapulmonary TB specimens.

An important exam point: a normal chest X-ray does not rule out spinal TB. Only about one-third to one-half of patients with Pott disease have concurrent active pulmonary disease. The primary pulmonary focus may have healed years before the skeletal disease manifests.

Laboratory markers such as ESR and CRP are nondiagnostic but are useful for monitoring treatment response. A falling ESR during therapy supports adequate treatment, while a persistently elevated or rising ESR warrants investigation for drug resistance, poor compliance, or an alternative diagnosis.

4. Management and Treatment

Pharmacological Treatment

The cornerstone of treatment is anti-tubercular chemotherapy, and the vast majority of patients respond to medical management alone.

Intensive Phase (2 months): Administer the four-drug combination daily:

Rifampicin: 10 mg/kg/day (max 600 mg)
Isoniazid: 5 mg/kg/day (max 300 mg)
Pyrazinamide: 25 mg/kg/day (max 2 g)
Ethambutol: 15 to 20 mg/kg/day (max 1.6 g)

Continuation Phase (10 to 16 months): Continue with two drugs:

Rifampicin: 10 mg/kg/day
Isoniazid: 5 mg/kg/day

The total duration for spinal TB is longer than standard pulmonary TB (6 months). WHO guidelines recommend a minimum of 12 months for bone and joint TB, and many clinical experts extend to 18 months, particularly for patients with large abscesses, slow radiographic response, or neurological involvement.

Monitoring during treatment:

Baseline and periodic liver function tests (Rifampicin, Isoniazid, and Pyrazinamide are all hepatotoxic)
Visual acuity and color vision testing for Ethambutol (risk of optic neuritis)
Monitor ESR/CRP for treatment response
Serial imaging (MRI or X-ray) every 3 to 6 months

Drug resistance: If GeneXpert shows rifampicin resistance, suspect multidrug-resistant TB (MDR-TB) and refer to a specialized center for a second-line regimen.

Surgical Indications

Surgery is not routine but is required in the following situations (remember the mnemonic for exam purposes):

The standard surgical approach is anterior decompression and debridement (the Hong Kong procedure, described by Hodgson and Stock) with bone grafting and instrumented stabilization. Anterior approach is preferred because the disease is predominantly in the anterior column. Posterior instrumentation may be added for additional stability, especially in the thoracolumbar spine.

Contraindications and Cautions

Pyrazinamide is contraindicated in severe hepatic disease and can cause hyperuricemia (monitor uric acid levels).
Ethambutol should be used cautiously in patients with renal impairment (dose adjustment required) and in young children where visual acuity cannot be reliably monitored.
Rifampicin is a potent CYP450 inducer and decreases the efficacy of oral contraceptives, warfarin, antiretroviral drugs, and many other medications. In HIV co-infected patients, coordinate with the antiretroviral regimen.
Isoniazid can cause peripheral neuropathy (prevent with pyridoxine) and hepatitis. Slow acetylators are at higher risk.
In pregnancy, Streptomycin is contraindicated (ototoxic to the fetus). The standard RHZE regimen is generally considered safe.

6. Traps and High-Yield Pearls

The single most common way students lose points on TB spondylitis questions is by failing to distinguish it from pyogenic spondylodiscitis. Both present with back pain and vertebral destruction, but the time course is the giveaway: a vignette describing a patient with weeks to months of insidious back pain, night sweats, weight loss, and imaging showing destruction of two contiguous vertebral bodies with a large paravertebral or psoas "cold" abscess is classic Pott disease. A pyogenic infection is acute, febrile, and rapid. If the vignette mentions a cold abscess or a gibbus deformity, the answer is TB spondylitis until proven otherwise.

Another frequent trap involves the chest X-ray. Students often look for concurrent pulmonary TB to "confirm" the diagnosis and eliminate the answer choice when the chest X-ray is normal. Remember that a normal chest X-ray does not exclude spinal TB. The primary pulmonary infection may have resolved years before skeletal manifestation.

A third testing point is the distinction between tumors and infection regarding disc space involvement. Vignettes that describe vertebral body destruction with a preserved disc space are pointing toward metastatic disease, not TB. TB destroys the disc because the infection spreads from the endplate across the disc; tumors do not cross the disc because avascular disc tissue resists tumor invasion.

On management questions, the core competency being tested is knowing that the duration of anti-TB therapy for bone and joint TB is longer than for pulmonary TB (12 to 18 months versus 6 months). Students who reflexively choose "6 months of RHZE then stop" will be marked wrong. Additionally, remember that most patients with Pott disease, even those with mild neurological deficits, respond to medical therapy alone. Surgery is reserved for patients who fail to improve on chemotherapy, who have severe or progressive neurological deterioration, or who have structural instability requiring stabilization. The default "next best step" for a newly diagnosed patient with Pott disease and no alarming neurological deficit is initiating anti-TB chemotherapy, not referral for surgery.

Finally, watch for questions that test drug side effects during long-term therapy. A patient on treatment who develops visual changes (decreased acuity or red-green color blindness) is being tested on ethambutol-induced optic neuritis. A patient developing tingling in the extremities is being tested on isoniazid-induced peripheral neuropathy (prevented by pyridoxine). A patient with jaundice during treatment requires immediate evaluation of liver function, and the hepatotoxic agents (Isoniazid, Rifampicin, Pyrazinamide) must be held until transaminases normalize.
