01Pathophysiology
Giardia lamblia exists in two forms that are central to understanding the disease. The cyst is the infectious form, environmentally hardy and resistant to standard chlorination of water. As few as 10 to 25 cysts are sufficient to establish infection, making it highly contagious in communal settings like day care centers. Cysts are ingested through contaminated water (classically, untreated stream or lake water) or through direct fecal-oral contact.
Once ingested, cysts pass through the acidic gastric environment and undergo excystation in the duodenum, releasing the motile trophozoite form. Trophozoites use a ventral adhesive disc to attach firmly to the brush border of enterocytes lining the duodenum and proximal jejunum. This attachment is purely superficial: trophozoites do not invade the intestinal mucosa. This is a critical concept because it explains why the diarrhea is non-bloody and why fecal leukocytes are absent. If a vignette describes bloody diarrhea or fecal WBCs, Giardia is not the answer.
The pathogenic mechanism centers on mechanical obstruction of the brush border and the subsequent villous atrophy that develops with chronic infection. Damaged villi lose their surface area and their enzyme-producing capacity, particularly disaccharidases such as lactase. This leads to secondary lactose intolerance, a finding that can persist even after eradication of the organism. The loss of absorptive surface also impairs fat absorption, producing the hallmark steatorrhea (foul-smelling, greasy, floating stools). Malabsorption of fat-soluble vitamins (A, D, E, K) and other nutrients drives the weight loss and failure to thrive seen in chronic pediatric cases.
The immune response to Giardia relies heavily on secretory IgA. This is why patients with IgA deficiency or common variable immunodeficiency (CVID) are predisposed to severe, prolonged, and recurrent giardiasis. When a vignette pairs chronic diarrhea with a known immunoglobulin deficiency, Giardia should be at the top of the differential.

In children, the chronic form is particularly high-yield. A vignette describing a child in a day care setting with failure to thrive, chronic diarrhea, and foul-smelling stools is a classic setup. In adults, the prototypical patient is a hiker or camper who drank from a stream and now presents weeks later with ongoing greasy diarrhea and bloating.

The workup for suspected giardiasis should begin with a stool antigen detection test (either ELISA or direct fluorescent antibody). This is the best initial test because it is non-invasive, requires only one stool sample, and has a sensitivity exceeding 95%. It is more reliable than a single stool O&P examination.
If antigen testing is unavailable, the classic approach is stool microscopy for ova and parasites. However, a single O&P specimen has a sensitivity of only about 50 to 70% because cyst shedding is intermittent. For this reason, guidelines recommend collecting three stool samples on different days. On microscopy, the trophozoite is described as pear-shaped with bilateral symmetry, two nuclei, and four pairs of flagella, giving it a characteristic "face-like" appearance. Cysts are oval and contain two to four nuclei.
A critical negative finding is that stool studies will show no red blood cells and no white blood cells. This is because Giardia does not invade the mucosa. If the question mentions fecal leukocytes or occult blood, the answer is not giardiasis.
Duodenal biopsy is the gold standard but is reserved for diagnostically challenging cases. Histology reveals trophozoites adherent to the villous surface and varying degrees of villous blunting. The string test is largely of historical interest now, but it still appears in vignettes as a distractor or a recognized sampling method.
When a vignette presents a patient with chronic non-bloody diarrhea, steatorrhea, and recent travel or day care exposure, and the question asks for the "next best step," the answer is stool antigen testing, not empiric treatment and not endoscopy.

Tinidazole is the preferred first-line agent for adults and older children because it achieves a cure rate above 90% with a single 2-gram dose, which greatly improves compliance. For exam purposes, tinidazole is the best answer when the question asks for "the most appropriate treatment."
Metronidazole remains a widely tested drug on exams. The classic regimen is 250 mg three times daily for 5 to 7 days. Patients must be counseled to avoid alcohol during and for at least 48 hours after completing the course because of the disulfiram-like reaction (severe nausea, vomiting, flushing, tachycardia). This drug interaction is a frequently tested concept.
In pregnant patients, both metronidazole and tinidazole are contraindicated in the first trimester due to potential teratogenicity. The safe alternative is paromomycin, a non-absorbable aminoglycoside that acts locally within the intestinal lumen and does not cross the placenta in clinically significant amounts. If a vignette describes a pregnant woman with giardiasis, paromomycin is the answer.
For pediatric patients, nitazoxanide is the preferred agent because it is FDA-approved for this age group, well-tolerated, and available as a pleasant-tasting suspension.
Asymptomatic carriers generally do not require treatment unless they are in a high-risk setting (food handlers, household contacts of immunocompromised individuals, or day care workers). However, if a vignette presents a symptomatic patient, always treat.
After treatment, patients with persistent symptoms should be evaluated for secondary lactose intolerance, which can last for weeks to months after the parasite is cleared. A lactose-free diet trial is appropriate before assuming treatment failure.

The single most important discriminator for Giardia on an exam is the combination of non-bloody, greasy diarrhea with an appropriate exposure history (contaminated water, day care, travel). When the stool has blood or the patient has fever with dysentery, look elsewhere.
06Traps and High-Yield Pearls
The most common way students lose points on giardiasis questions is by selecting an invasive organism when the vignette clearly describes a non-invasive process. The absence of blood, the absence of fecal leukocytes, and the presence of steatorrhea are all pointing toward a surface-dwelling organism that causes malabsorption, not mucosal destruction. If you see "foul-smelling, greasy, floating stools" and there is no blood, think Giardia before anything else.
A second common trap involves the diagnostic test. Students often select "stool O&P" as the best initial test, but a single stool O&P has poor sensitivity due to intermittent shedding. The better answer on modern exams is stool antigen testing, which requires only one sample and has superior sensitivity. Reserve stool O&P for settings where antigen testing is not available, and remember the "rule of three" (three specimens on separate days) if microscopy is the only option.
Another frequently tested pitfall is the pregnant patient with giardiasis. Metronidazole is the reflexive answer for many students, but it is avoided in the first trimester. The correct choice in pregnancy is paromomycin.
Finally, do not forget the IgA deficiency connection. A vignette that pairs recurrent or refractory giardiasis with a history of sinopulmonary infections and low immunoglobulin levels is testing your ability to recognize selective IgA deficiency or CVID as the underlying predisposition. The parasite question is straightforward; the real question is whether you can identify the immune defect.
The core competency being tested across giardiasis questions is your ability to recognize a non-invasive, malabsorptive diarrheal illness, select the correct diagnostic study, choose the right drug for the right patient (including pregnant women and children), and connect chronic or recurrent disease to an underlying immunodeficiency.
