1. Pathophysiology

ADPKD

ADPKD is a ciliopathy. The proteins polycystin-1 (encoded by PKD1) and polycystin-2 (encoded by PKD2) form a receptor-channel complex localized on the primary cilium of renal tubular epithelial cells. This complex acts as a mechanosensor for urinary flow, regulating intracellular calcium signaling. When either polycystin is dysfunctional, the cell loses its ability to sense flow, and intracellular calcium drops.

Low intracellular calcium triggers increased cyclic AMP (cAMP) accumulation. Elevated cAMP drives two pathological processes simultaneously: it stimulates cell proliferation (via the MAPK/ERK pathway) and promotes fluid secretion (via CFTR chloride channels on the apical membrane). Together, these processes cause the tubular epithelium to proliferate and secrete fluid into progressively expanding cysts. This is why the kidneys grow relentlessly over decades. Each cyst eventually pinches off from the parent tubule and becomes an autonomous, fluid-filled sac that continues to expand.

PKD1 mutations produce a more severe phenotype than PKD2. Patients with PKD1 develop more cysts earlier, have larger kidneys at diagnosis, and reach end-stage kidney disease (ESKD) at a median age of approximately 54 years. Patients with PKD2 reach ESKD at a median age of approximately 74 years. Truncating PKD1 mutations carry the worst prognosis of all, and this genotype-phenotype correlation is central to the Mayo Imaging Classification and PROPKD scoring used to identify rapid progressors.

The cyst expansion compresses normal renal parenchyma, activates local inflammation and fibrosis, and progressively destroys functional nephrons. This is why patients develop hypertension early (due to activation of the renin-angiotensin-aldosterone system from ischemia of compressed parenchyma), flank pain (from cyst stretching the renal capsule or hemorrhage into a cyst), hematuria (from cyst rupture into the collecting system), and nephrolithiasis (from urinary stasis and low urinary citrate).

The same polycystin defect exists in cholangiocytes and other ductal epithelia, which is why hepatic cysts are the most common extrarenal manifestation (found in up to 80% of patients by age 60), followed by pancreatic cysts, splenic cysts, and seminal vesicle cysts. The vascular wall also expresses polycystins: loss of vascular polycystin weakens the tunica media of cerebral arteries, leading to intracranial (berry) aneurysms in approximately 8 to 10% of ADPKD patients. This is the link between ADPKD and subarachnoid hemorrhage, and it is one of the most high-yield associations on any exam.

Additional cardiovascular manifestations include mitral valve prolapse (in up to 25% of patients) and colonic diverticulosis, both attributable to connective tissue weakness from the underlying ciliopathy.

ARPKD

ARPKD is caused by mutations in PKHD1, which encodes fibrocystin, another ciliary protein expressed in renal collecting duct cells and biliary epithelium. Unlike ADPKD, where cysts arise from all segments of the nephron and become large and round, ARPKD cysts represent fusiform dilation of the collecting ducts. On gross examination, the kidneys are bilaterally enlarged, smooth-surfaced, and sponge-like with radially oriented, elongated cystic channels.

The critical distinction for exam purposes is the obligatory hepatic component. In ARPKD, the ductal plate malformation of the intrahepatic bile ducts produces congenital hepatic fibrosis in essentially all patients, and this can progress to portal hypertension with splenomegaly, esophageal varices, and thrombocytopenia. A subset of patients develop Caroli disease (segmental dilation of intrahepatic bile ducts), predisposing to recurrent ascending cholangitis.

In its most severe perinatal form, the kidneys are so enlarged in utero that they compress the fetal abdomen, reduce amniotic fluid production, and cause oligohydramnios. This leads to Potter sequence: pulmonary hypoplasia, flattened facies, and limb deformities. Pulmonary hypoplasia is the leading cause of death in the neonatal period.

Best Initial Test: Renal Ultrasound. Ultrasound is inexpensive, non-invasive, and widely available. In a patient with a positive family history, ultrasound is the first-line diagnostic study. The diagnosis is based on the Pei-Ravine criteria (unified criteria for both PKD1 and PKD2):

Ages 15 to 39: $\geq$3 total renal cysts (unilateral or bilateral). A positive predictive value of 100%.
Ages 40 to 59: $\geq$2 cysts in each kidney.
Ages 60 and older: $\geq$4 cysts in each kidney.

Exclusion is also age-dependent: in patients aged 40 and older, the presence of $\leq$2 total cysts has a negative predictive value of essentially 100%. In younger patients, a normal ultrasound does not definitively exclude the disease because cysts may not yet have developed.

Most Accurate Test: MRI. MRI provides the most precise measurement of total kidney volume (TKV), which is the single best imaging biomarker for disease progression. The Mayo Imaging Classification (Class 1A through 1E) uses height-adjusted TKV and age to stratify patients into risk categories for rapid progression. This classification directly determines eligibility for tolvaptan therapy. MRI is also superior for detecting small cysts and for evaluating polycystic liver disease.

MRA for Intracranial Aneurysm Screening. Intracranial berry aneurysms are the most feared extrarenal complication of ADPKD. MRA (magnetic resonance angiography) of the circle of Willis is the preferred screening modality. Intracranial aneurysms are found in approximately 10% of asymptomatic ADPKD patients on screening, with a higher prevalence in those with a family history of aneurysm or subarachnoid hemorrhage. Screening is recommended for patients with:

Family history of intracranial aneurysm or subarachnoid hemorrhage
High-risk occupations (e.g., pilots)
Prior to elective surgery
Patient anxiety warranting reassurance

Genetic Testing. Genetic testing is not routinely needed when the clinical and imaging picture is clear. It becomes valuable in:

Patients younger than 15 with equivocal ultrasound
Patients without a family history (de novo mutations account for ~10%)
Evaluation of potential living kidney donors from affected families
Distinguishing ADPKD from other cystic kidney diseases

In ARPKD, the diagnostic approach differs. Prenatal ultrasound may reveal enlarged echogenic kidneys and oligohydramnios. Postnatal ultrasound shows bilaterally enlarged, hyperechogenic kidneys with poor corticomedullary differentiation and no visible discrete round cysts (unlike ADPKD). Genetic testing for PKHD1 mutations is confirmatory. Liver biopsy demonstrating congenital hepatic fibrosis with ductal plate malformation supports the diagnosis.

Blood Pressure Control. Hypertension is often the earliest clinical finding and a primary driver of cardiovascular morbidity and accelerated renal decline. The preferred agents are ACE inhibitors or ARBs, targeting a blood pressure of $\leq$110/75 mmHg in young, healthy patients aged 18 to 50 with $eGFR > 60$ mL/min/1.73 m$^2$. This aggressive target comes from the HALT-PKD trial, which demonstrated that intensive blood pressure lowering in early ADPKD reduces the rate of kidney volume growth and urinary albumin excretion. In older patients or those with more advanced CKD, a standard target of <130/80 mmHg is appropriate.

ACE inhibitors and ARBs are preferred because the RAAS is pathologically activated by cyst-mediated parenchymal ischemia. Dual RAAS blockade (ACEi plus ARB) is not recommended. The HALT-PKD Trial B showed no benefit of combination therapy.

Tolvaptan. Tolvaptan is a selective vasopressin V2 receptor antagonist and is the only pharmacological agent proven to slow disease progression in ADPKD. It is recommended in adults with ADPKD and $eGFR \geq 25$ mL/min/1.73 m$^2$ who are at risk of rapidly progressive disease and have no absolute contraindications. The mechanism is direct: by blocking the V2 receptor in the collecting duct, tolvaptan reduces intracellular cAMP accumulation, thereby slowing both cyst cell proliferation and fluid secretion.

The dosing regimen is a split-dose schedule. The initiation dose is 45 mg in the morning and 15 mg in the afternoon/evening. Titration proceeds to 60/30 mg and then to the target dose of 90/30 mg, with each step separated by at least one week. The higher morning dose maximizes daytime V2 receptor suppression, while the lower evening dose reduces nocturia. Down-titration to 30/15 mg or 15/15 mg is permitted for tolerability.

Key side effects and monitoring requirements:

Aquaresis (polyuria and polydipsia): The most common side effect. Patients may produce 6 to 12 liters of dilute urine daily. Adequate fluid replacement is essential.
Hepatotoxicity: The most serious risk. Liver function tests (AST, ALT, total bilirubin) must be checked at baseline, monthly for the first 18 months, then every 3 months thereafter. Tolvaptan must be stopped if ALT or AST rises above 3 times the upper limit of normal or if bilirubin rises above 2 times the upper limit.
Contraindications: Pregnancy or breastfeeding, inability to maintain adequate hydration, urinary tract obstruction, concurrent strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin), and liver disease other than uncomplicated polycystic liver disease.

A "sick day plan" is required for all patients on tolvaptan: patients should skip doses whenever there is a transient risk of volume depletion (vomiting, diarrhea, fasting for procedures).

Increased Water Intake. Patients should be counseled to drink 2 to 3 liters of water per day unless contraindicated. The rationale is that increased water intake suppresses endogenous vasopressin release, partially mimicking the effect of tolvaptan. However, increased water intake alone is not a substitute for tolvaptan in patients with documented rapid progression.

ARPKD Management. There is no disease-modifying therapy for ARPKD. Management is entirely supportive:

Neonatal period: ventilatory support for pulmonary hypoplasia; management of respiratory failure is the immediate priority.
Hypertension: ACE inhibitors or ARBs are used, though data are extrapolated from adult CKD guidelines.
Portal hypertension: management of variceal bleeding (endoscopic banding), ascites, and cholangitis (antibiotics for Caroli-related infections).
Renal replacement therapy for progressive CKD.
Combined liver-kidney transplantation is the definitive treatment for patients with both ESKD and severe hepatic complications.

5. Differential Diagnosis and Distractors

The two distractors that appear most frequently on exams:

Simple renal cysts vs. ADPKD: The vignette gives you an older adult with a few cysts found incidentally. If the kidneys are normal in size, there is no family history, and renal function is preserved, the answer is simple cysts. ADPKD requires bilateral enlarged kidneys with multiple cysts meeting age-based criteria.
Acquired cystic disease vs. ADPKD: Both have bilateral cysts, but acquired cystic disease always occurs in the setting of longstanding dialysis, and the kidneys are small. ADPKD kidneys are always large.

6. Traps and High-Yield Pearls

The single most common way students get PKD questions wrong is by failing to connect the extrarenal associations. The test-writer is not just testing whether you know ADPKD causes kidney cysts. They are testing whether you understand the systemic nature of the ciliopathy. Expect a two-step vignette: a patient with known ADPKD who presents with a sudden severe headache, neck stiffness, and photophobia. The tested concept is that ADPKD is a risk factor for intracranial berry aneurysm leading to subarachnoid hemorrhage. If you do not make the connection, you will pick the wrong diagnosis.

A second common trap involves cyst infection vs. cyst hemorrhage. Both cause flank pain and fever. The distinguishing feature is that cyst infection shows a leukocytosis, positive blood cultures, and sometimes a rim-enhancing lesion on CT, while cyst hemorrhage presents with acute flank pain and hematuria but without infectious signs. Critically, the treatment of cyst infection requires lipophilic antibiotics (fluoroquinolones, TMP-SMX) because hydrophilic drugs such as penicillins and cephalosporins do not penetrate the cyst wall effectively.

A third trap is the tolvaptan liver monitoring question. The vignette may describe a patient on tolvaptan who develops elevated transaminases. The correct answer is to stop the drug and monitor, not to add hepatoprotective agents or "watch and wait." Monthly LFT monitoring for the first 18 months is a testable fact.

For ARPKD, the trap is confusing it with ADPKD in a neonate. If a newborn presents with bilateral enlarged kidneys, oligohydramnios, and pulmonary hypoplasia, the answer is ARPKD, not ADPKD. Yes, ADPKD can rarely present in neonates, but the classic exam presentation of a neonate with Potter sequence and congenital hepatic fibrosis is ARPKD until proven otherwise.

Finally, remember the inheritance pattern trap. ADPKD is autosomal dominant with high penetrance: each child of an affected parent has a 50% chance of inheriting the disease. ARPKD is autosomal recessive: carrier parents are unaffected, and each child has a 25% chance of being affected. The vignette will often give you a family tree and ask you to identify the inheritance pattern, or it will describe unaffected parents with a severely affected child to point you toward ARPKD.

The core competency being tested across all PKD questions is your ability to match the inheritance pattern to the age of presentation, connect the renal disease to its extrarenal complications, and sequence the diagnostic and therapeutic interventions correctly (ultrasound first, MRI for risk stratification, genetic testing for equivocal cases, tolvaptan for rapid progressors with preserved eGFR, and screening MRA for aneurysms when family history is positive).
