Blood cultures and CBC are obtained not to diagnose TTN but to exclude neonatal pneumonia and early-onset sepsis, which can present identically in the first hours of life. This is why empiric antibiotics are frequently started pending culture results at 48 hours.

The classic vignette setup is: a term neonate born via elective cesarean section (no labor) who develops tachypnea within the first few hours of life, has mild oxygen requirements, and a CXR showing perihilar streaking or fluid in the fissures. When this constellation appears, TTN is the answer.

The most dangerous distractor is RDS. Students see "respiratory distress in a newborn" and reflexively choose surfactant deficiency. The discriminator is gestational age: RDS is a disease of prematurity, while TTN is a disease of term or near-term infants. If the vignette states 38 weeks and cesarean delivery, that is TTN, not RDS.

Furosemide is a recurring trap answer in management questions. The reasoning ("there is excess fluid, so give a diuretic") sounds logical but is clinically incorrect. No pharmacologic intervention has been shown to accelerate resolution of TTN. The answer is always supportive care.
