1. Pathophysiology

IUGR is defined as a fetus with an estimated weight below the 10th percentile for gestational age, accompanied by evidence of pathologic growth restriction. It is important to distinguish IUGR from constitutionally small for gestational age (SGA). A constitutionally small fetus has normal Doppler studies, normal amniotic fluid, and follows a consistent growth trajectory. In contrast, true IUGR implies a fetus that has failed to reach its genetic growth potential due to a pathologic process, and this is often confirmed by abnormal Doppler velocimetry or declining growth trajectory on serial scans.

The pathophysiology diverges based on the timing of the insult, which produces two distinct phenotypes. Symmetric IUGR results from an early first-trimester insult that disrupts cellular hyperplasia (the phase when organ growth depends on increasing cell number). Because the insult occurs early, all organs are proportionally small, including the head. The diagnostic process for IUGR begins at the prenatal visit. Fundal height measurement is the screening tool used at every visit after 20 weeks. A discrepancy of 3 cm or more between fundal height (in centimeters) and gestational age (in weeks) should prompt an ultrasound. This is the cornerstone of surveillance and is the only Doppler modality that has been shown in randomized trials to reduce perinatal mortality when used for IUGR monitoring. A normal umbilical artery Doppler in the setting of EFW below the 10th percentile may indicate constitutionally small fetus or mild restriction with intact placental function. Progressive elevation of the S/D ratio indicates increasing placental vascular resistance. Absent end-diastolic flow (AEDF) indicates that roughly 60 to 70 percent of placental vasculature is obliterated, and reversed end-diastolic flow (REDF) indicates even more advanced disease.

In late-onset IUGR (32 weeks or beyond), the umbilical artery Doppler may remain normal despite genuine growth restriction. In these cases, the cerebroplacental ratio (CPR) becomes the earliest and most sensitive marker. There is no pharmacologic treatment that reverses established IUGR. Management is entirely surveillance-based, aimed at optimizing the timing of delivery to balance the risks of prematurity against the risk of stillbirth.

Antenatal corticosteroids (betamethasone 12 mg intramuscularly, two doses 24 hours apart) should be administered when preterm delivery is anticipated between 24 and 34 weeks of gestation. This reduces the risk of neonatal respiratory distress syndrome, intraventricular hemorrhage, and necrotizing enterocolitis. A single rescue course may be considered if the initial course was given more than 14 days prior and delivery remains imminent before 34 weeks.

Magnesium sulfate for neuroprotection (4 to 6 g intravenous loading dose) should be administered when delivery is expected before 32 weeks, as it reduces the risk of cerebral palsy in the preterm neonate.

The mode of delivery depends on fetal tolerance of labor. Vaginal delivery is not contraindicated in IUGR, but these fetuses have diminished reserve and are at higher risk of non-reassuring fetal heart rate tracings during labor. Continuous electronic fetal monitoring is required. Cesarean delivery is indicated for standard obstetric indications or if the fetus cannot tolerate labor.

Regarding modifiable risk factors, smoking cessation is the single most impactful intervention that can be offered to reduce progression. Low-dose aspirin (150 mg daily, started before 16 weeks) is used for prevention in patients at high risk for preeclampsia and placental insufficiency, but it does not treat established IUGR.

After delivery, the neonate is at risk for hypoglycemia (depleted glycogen stores), hypothermia (reduced subcutaneous fat), polycythemia (chronic hypoxia-driven erythropoietin elevation), and necrotizing enterocolitis. Early feeding and glucose monitoring are essential in the immediate neonatal period.

6. Traps and High-Yield Pearls

The single most common trap in exam questions on IUGR is confusing constitutionally SGA with pathologic IUGR. A vignette describing a small fetus with normal Doppler studies, normal amniotic fluid, and small parental stature is testing whether you know that this fetus does not need early delivery or aggressive intervention. The correct answer is continued routine surveillance. Conversely, a vignette that emphasizes oligohydramnios or abnormal Doppler findings is steering you toward pathologic IUGR and asking you to determine the next step in surveillance or timing of delivery.

Another frequently tested concept is the sequence of Doppler deterioration. Students often jump to immediate delivery when they see "IUGR" in a question stem, but the exam tests your ability to match the degree of Doppler abnormality to the appropriate gestational age for delivery. Remember the progression: elevated S/D ratio, then absent end-diastolic flow, then reversed end-diastolic flow, then abnormal ductus venosus. Each step escalates the urgency and moves the delivery threshold earlier.

The brain-sparing effect is a favorite test-writer concept. A question may describe a fetus with a normal-sized head but small abdominal circumference and ask you to identify the mechanism. The answer is preferential shunting of blood to the brain at the expense of splanchnic organs. This also explains why abdominal circumference is more sensitive than head circumference for detecting asymmetric IUGR.

Be alert for vignettes describing early symmetric IUGR with structural anomalies. The test is not asking about placental insufficiency. It is asking you to recognize that the next step is genetic testing (amniocentesis), not Doppler surveillance. The etiology drives the workup.

Finally, know that low-dose aspirin is preventive, not therapeutic. A question that presents a patient already diagnosed with IUGR at 28 weeks and offers aspirin as an answer choice is a distractor. Aspirin must be started before 16 weeks in at-risk patients to be effective. Once IUGR is established, management is surveillance and timed delivery, not pharmacotherapy.
