01Background and Regulatory Framework
The Indonesian national health insurance program is operated by BPJS Kesehatan and mandated by Undang-Undang No. 24 Tahun 2011. The primary regulation governing contribution amounts is Peraturan Presiden (Perpres) No. 82 Tahun 2018 on Health Insurance, which was updated through Perpres No. 64 Tahun 2020. Additional payment mechanism rules are laid out in Perpres No. 63 Tahun 2022.
The most recent amendment is Perpres No. 59 Tahun 2024, ratified on May 8, 2024, which serves as the third amendment to Perpres No. 82/2018. This regulation introduced the concept of Kelas Rawat Inap Standar (KRIS), a standard inpatient class that replaces the old Class 1, 2, and 3 system.
It is important to understand that iuran (contributions) and denda (penalties) are governed by different mechanisms. Contributions are fixed monthly obligations, while penalties arise only when a reactivated participant uses inpatient services within a defined grace period. A common misconception is that late payment automatically triggers a monetary fine; in reality, the penalty is tied to a clinical event (inpatient care), not the administrative act of paying late.
02Iuran BPJS Kesehatan
A. Penerima Bantuan Iuran (PBI)
Item | Detail |
|---|---|
Who qualifies | Citizens classified as underprivileged (fakir miskin/tidak mampu) listed in the Data Terpadu Kesejahteraan Sosial (DTKS) of the Ministry of Social Affairs |
Contribution amount | Rp 42,000 per person per month, entirely funded by the government through APBN or APBD |
Out-of-pocket cost | Rp 0 |
B. Pekerja Penerima Upah (PPU) -- Government and Private Sector
Item | Detail |
|---|---|
Who qualifies | Civil servants (PNS), TNI/Polri members, state officials, non-civil-servant government employees, and employees of BUMN, BUMD, and private companies |
Total contribution | 5% of monthly salary (gaji pokok + tunjangan tetap) |
Employer share | 4% |
Employee share | 1% (deducted from salary) |
Salary ceiling | The upper limit for calculation is Rp 12,000,000; any salary above this figure is still calculated based on Rp 12 million |
Salary floor | The lower limit is the local UMK/UMP (district/provincial minimum wage) |
Coverage | The 5% contribution covers up to 5 family members: the worker, spouse, and up to 3 children |
Additional family members | For the 4th child onward, parents, and parents-in-law, an additional 1% of salary per person per month is charged, paid by the employee |
Calculation example: A private-sector employee earns Rp 10,000,000/month.
Total iuran = 5% x Rp 10,000,000 = Rp 500,000
Employer pays: 4% x Rp 10,000,000 = Rp 400,000
Employee pays: 1% x Rp 10,000,000 = Rp 100,000
If an employee earns Rp 20,000,000, the calculation is still based on the Rp 12,000,000 ceiling, so the maximum monthly deduction from the employee's salary is 1% x Rp 12,000,000 = Rp 120,000.
C. Pekerja Bukan Penerima Upah (PBPU) and Bukan Pekerja (BP)
These are self-employed individuals, freelancers, entrepreneurs, investors, and other persons not receiving a regular salary from an employer. They select their own class.

03Transition to KRIS (Kelas Rawat Inap Standar)
This is an important contextual update. Through Perpres No. 59/2024, the government officially replaced the Class 1, 2, 3 system with KRIS, which has been in effect starting 2025 and continuing through 2026.
Under KRIS, all hospitals partnering with BPJS Kesehatan must meet 12 facility criteria, including a maximum of 4 beds per room, minimum 1.5-meter distance between beds, an ensuite bathroom meeting accessibility standards, oxygen outlets, and room temperature maintained at 20 to 26 degrees Celsius.
However, here is the exam-relevant point: although the inpatient service system has transitioned to KRIS, the contribution amounts have not been updated. The government has not yet set a new unified rate, so until further notice, contribution amounts remain identical to the pre-KRIS class-based figures. This means the Class 1/2/3 contribution tiers are still the administrative basis for payment even as the clinical service is being standardized.
04Payment Rules and Deactivation Mechanism
Payment Deadline
Rule | Detail |
|---|---|
Due date | The 10th of every month |
If the 10th is a weekend/holiday | Participants are advised to pay earlier; the system records the actual transaction date, not the intended payment date |
Late payment fine | No late payment fine has been applied since July 1, 2016 |
Status change upon default | If payment is made after the due date, the participant's status becomes temporarily nonactive starting the 11th of the current month |
Deactivation and Reactivation
When a participant fails to pay, the membership is temporarily suspended. If the participant defaults for more than one month, arrears accumulate. The participant must settle all outstanding contributions to reactivate their card.
The maximum number of months of arrears that can be settled in one payment is 12, even if the participant has defaulted for longer than that.
For new registrants, the card becomes active 14 working days after the first payment. For reactivation after arrears, the card becomes active within 1x24 hours after payment is recorded.
05Denda Pelayanan BPJS Kesehatan
This is the most commonly tested and misunderstood part. The penalty is not a late-payment fee. It is a service-utilization penalty tied to inpatient care.
Conditions for Penalty Imposition
Three conditions must ALL be met for the penalty to apply. First, the participant previously had unpaid arrears. Second, the participant has settled all outstanding contributions. Third, the participant uses inpatient services within 45 days of membership reactivation.
If any one of these conditions is absent, no penalty is charged.
Penalty Formula
Component | Value |
|---|---|
Rate | 5% (fixed) |
Base | Initial inpatient diagnosis cost (biaya diagnosa awal pelayanan rawat inap) based on INA-CBGs tariff |
Multiplier | Number of months of arrears |
Maximum months counted | 12 months |
Maximum penalty cap | Rp 30,000,000 |
Worked Examples
Example 1: A participant defaulted for 5 months, then settled all arrears. Two weeks later, they are hospitalized with an initial diagnosis cost of Rp 10,000,000.
Penalty = 5% x Rp 10,000,000 = Rp 500,000 per month of arrears. Multiply by 5 months = Rp 2,500,000.
The participant must pay Rp 2,500,000 as a penalty on top of their settled arrears.
Example 2: A participant defaulted for 10 months, then settled arrears. They are hospitalized within 45 days with an initial diagnosis cost of Rp 10,000,000.
Penalty = 5% x Rp 10,000,000 x 10 months = Rp 5,000,000.
Example 3 (Testing the cap): A participant defaulted for 15 months with diagnosis cost Rp 60,000,000.
The months counted are capped at 12: 5% x Rp 60,000,000 x 12 = Rp 36,000,000. But the absolute cap is Rp 30,000,000, so the penalty charged is Rp 30,000,000.
Who Pays the Penalty?
Participant Type | Penalty Responsibility |
|---|---|
PBPU / BP (Independent) | The participant bears the penalty themselves |
PPU (Employee) | The employer bears the penalty, since the arrears arose from the employer's failure to remit contributions |
06Distractors
"Late payment itself triggers a monetary fine"
Why Students Choose It
Intuitive assumption that late = fine
Key Discriminator (Correct Fact)
No late payment fine has existed since July 1, 2016. Penalty applies only when inpatient care is accessed within 45 days of reactivation
"The penalty is 2.5% of the total hospital bill"
Why Students Choose It
Confusion with older regulations (pre-2020)
Key Discriminator (Correct Fact)
Current rate is 5% of the initial diagnosis cost (not total bill), multiplied by months of arrears
"Maximum penalty is Rp 20,000,000"
Why Students Choose It
An older BPJS Kesehatan social media post mentioned Rp 20 million as the cap, which may reflect a superseded regulation
Key Discriminator (Correct Fact)
Under Perpres No. 64/2020, the current cap is Rp 30,000,000
"The 45-day rule starts from the date of payment"
Why Students Choose It
Confusion about the trigger point
Key Discriminator (Correct Fact)
The 45-day window starts from the date the membership is reactivated, which is within 1x24 hours of payment
"PBPU participants can only pick Class 3"
Why Students Choose It
Confusion between PBI and PBPU
Key Discriminator (Correct Fact)
PBPU participants choose their own class (1, 2, or 3); it is PBI participants who receive government-designated coverage
"With KRIS, all participants now pay the same amount"
Why Students Choose It
Assumption that equal service = equal contribution
Key Discriminator (Correct Fact)
KRIS standardizes the inpatient facility, but the contribution tiers remain unchanged during the transition period
"PPU participants choose their own class"
Why Students Choose It
Mixing up PPU and PBPU rules
Key Discriminator (Correct Fact)
For PPU participants, the class is determined by BPJS Kesehatan based on the employee's salary, not chosen by the participant

07Traps and High-Yield Pearls
The most common way students get questions on this topic wrong is by conflating late-payment consequences with the inpatient service penalty. A vignette might describe a patient who paid late, then visited a puskesmas for outpatient care two weeks after reactivation. Students who select "the patient must pay a 5% penalty" are wrong, because the penalty only applies to rawat inap (inpatient care), not outpatient visits.
Another trap involves the penalty calculation cap. A vignette may present a patient with 18 months of arrears and a very high diagnosis cost. Students who multiply 5% x cost x 18 months will produce an absurdly large number. The correct answer requires capping the months at 12 and the total penalty at Rp 30,000,000.
For PPU-related questions, remember that the employer, not the employee, is liable for the penalty when the employer is the one who failed to remit iuran. A clinical vignette might present a hospitalized factory worker whose company neglected payments. The question tests whether you know the penalty burden shifts to the employer.
Regarding KRIS, the exam may try to trick you into thinking that KRIS eliminates contribution differences. It does not. KRIS eliminates facility differences in inpatient rooms. The administrative contribution structure (Class 1/2/3 pricing for PBPU, percentage-based for PPU) remains intact until a new presidential regulation formally replaces it.
Finally, a core principle: the tanggal 10 rule is non-negotiable. Even if the 10th falls on a weekend, the system counts the actual transaction date. There is no grace period. Missing the 10th by even one day results in nonactive status as of the 11th (or the 1st of the following month, depending on the source's framing of the same rule). This payment discipline question is straightforward but commonly tested as a factual recall item.
